Acknowledgement and Assumption of Risk

I acknowledge that I have voluntarily chosen to receive massage therapy, facial, and/or eyelash lift and tint services. I understand that these services are not a substitute for medical treatment or medications. I am aware that the therapist does not diagnose illnesses or injuries, prescribe medications, and that spinal manipulations are not part of massage therapy. I am aware that the treatment is holistic in nature and does not offer any guaranteed outcomes. I understand that some techniques may not be appropriate for certain medical conditions and will inform the therapist of all known medical conditions and any changes in my health status.

Health and Safety

I understand that failure to disclose pertinent medical information could result in adverse reactions to treatment. I understand the importance of informing Joana of any changes in my medical status. I agree to inform Joana if I experience any discomfort during the session so that the pressure/methods can be adjusted to my level of comfort.

Contraindications

I understand that certain medical conditions may be contraindicated for massage, facial, or eyelash treatments. In the event that I have a condition that is contraindicated, I agree to provide a doctor's clearance if required.

Cancellation Policy

I understand the business's cancellation policy and agree to provide notice if I need to reschedule or cancel my appointment to avoid any potential cancellation fees.

Liability Release

I hereby agree to release and hold harmless Beauty Balance by Jo, its owner(s) and therapist(s) from any and all liability, claims, demands, actions, and causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in such activity, or while in, on or upon the premises where the activity is being conducted.

Acknowledgment of Understanding

I acknowledge that by proceeding with the booking of massage, facial, or eyelash treatment services, I have read and fully understood the waiver of liability, assumption of risk, and indemnity agreement as presented on this website. My continuation with the booking process serves as my agreement to these terms and conditions in their entirety, indicating a complete and unconditional release of all liability to the fullest extent allowed by law.
